A Clinical Observation of Uretural Injury


Abstract
A clinical observation vas made on the injuries of urethra of the in patient at department of urology. Chosun university hospital during the period from Jan. 1981 to Dec. 1986. The result were as follows:
1. All of 1686 cases hospitalized, 75 cases were injury of urethra giving tthe rate of 4.45%.
2. The most prevalent group ranged from 41 to 50 years (28%) the sex ratio. male to female was 74:1. 3. The most frequent involved site was anterior urethra (53.3%) and the others were the posterior urethra
(45.4%) and female urethra (1.3%).
4. The most common cause of urethral injury was falling and straddle injury (48%). and following by traffic accident (30.7% .iatrogenic injury (10.7%) etc.
5. Among the cases of urethral injury the pubic bone and pelvic bone fracture were associated in 20 cases (38.5%).
6. The incidence of impotence was case of all posterior total rupture.
7. The symptoms of urethral injury were urethral bleeding 41 (45.6%) dysuria 21 case (23.4%) acute urinary retension 13 case (14.8%) and others.
8. Initial management of urethral injury consisted of suprapubic cystostomy 41 cases (54.6%) Indwell
ing catherter 23 cases (30.7%) observation 8 cases (10.7%) and immediate urethroplasty 3 cases (4%). 9. The incidence of infection was 26 cases (34.6%) among the 26 cases the total rupture were 22 cases
(15.4%)
10. Of the 37 case of urethroplasty: ant urethroplasty was performed in 21 cases of which a perineal urethroplasty in 20 cases realignment 1 cases, post urethroplasty was performed in 16 cases, of with a perineal urethroplasty 12 cases transpublic urethroplasty 3 cases , interlocking i cases.
11. The complication of anterior urethral injury were urethral strictue 23 cases incontinence 4 cases prostatitis 2 cases, the complication of posterior urethral injury were urethral sricture 13 cases incontinence 7 cases impotence 5 cases.
12. Delayed transperineal urethroplasty was less complicated than inmediated urethral realignment with interlocking sounds.
